It was developed in the 19th century by the Belgian mathematician Adolphe Quetelet as a way to study the 'average man' across a population, not as an individual diagnostic tool, and it was only adopted much later, in the mid-20th century, as a standardized screening measure in medicine and public health. Its enduring appeal is simplicity: unlike body fat percentage (which requires specialized equipment like calipers, bioelectrical impedance scales, or DEXA scans to measure directly), BMI needs only a scale and a tape measure or stadiometer, making it cheap and practical to calculate for large populations and in routine clinical settings.
In metric units, BMI is weight in kilograms divided by height in meters squared (kg/m²) — for example, a 70kg person who is 1.75m tall has a BMI of 70 ÷ (1.75 × 1.75) = 70 ÷ 3.0625 ≈ 22.9. In imperial units, the formula is adjusted with a conversion constant: BMI = 703 × weight in pounds ÷ (height in inches)², where 703 converts the pounds-per-square-inch relationship into the equivalent kilograms-per-square-meter scale so that the resulting BMI number matches what the metric formula would produce for the same person. The Normal category (18.5 to 24.9) is the range associated with the lowest average health risk in most large population studies, though this doesn't mean every individual within this range is automatically healthy, nor that everyone outside it is automatically unhealthy — individual health depends on many factors BMI alone doesn't capture. The Overweight category (25 to 29.9) and Obese category (30 and above) are both associated with progressively increased average risk of conditions like type 2 diabetes, cardiovascular disease, and certain other health issues at a population level, which is why these categories are often used as a screening trigger for further individual evaluation, rather than as a standalone diagnosis on their own.
BMI's core limitation is structural: it's calculated purely from height and weight, with no way to distinguish how much of that weight comes from muscle versus fat versus bone versus water, even though these have very different health implications at the same total weight. A muscular athlete can register as Overweight or even Obese by BMI despite having low body fat and excellent cardiovascular fitness, while someone with a Normal BMI can still carry a health-relevant amount of body fat if they have low muscle mass — a combination sometimes informally called 'skinny fat.' BMI also can't capture where fat is distributed on the body, even though abdominal (visceral) fat carries a different health risk profile than fat distributed elsewhere at the same total body fat percentage. These are well-documented, widely acknowledged limitations within the medical and public health community itself, not a fringe criticism — which is exactly why BMI is generally treated as a useful first-pass screening tool rather than a complete individual health assessment on its own.
